Health Insurance (Medical and Vision)


Regular employees who work twenty (20) hours per week or more are eligible for employee and dependent insurance coverage. Insurance premiums for part time positions are pro-rated. Kaiser starts the first of the month following your hire date, even if you started on the 1st. MODA medical starts on the 1st of the month following your hire date, unless you started on the 1st. The city offers the following options for health insurance coverage.

Moda Preferred 200 Plan


Available to BPA and management employees. Employees who select this option have the option of choosing a health care provider from an established list of primary care physicians (PCP) and preferred specialists. The list includes chiropractors. This plan is a self-referral plan, but a PCP is recommended. Benefits, for the most part, are payable at 70%. Employees pay a 10% co-payment at each visit and there is a deductible of $200 ($600 family).
Prescriptions purchased from retailers are reimbursed at 90% with submission of receipt and claim form. Mail order prescriptions are $30 for a 90-day supply (generic) and $50 for a 90-day supply (brand) plus the cost difference between generic and brand.

Moda Preferred 250


Available to SEIU and management employees. Employees are encouraged to select a primary care physician (PCP) from an established list of preferred providers. This is a self referral plan. Employees pay a $20 co-payment at each visit. If employees do not go through their PCP coverage is at 70% with a $300 deductible $900 family deductible). This plan has co-pays, cost sharing percentages (90% in network; 70% out of network). The deductible is $250 ($750 for family).

HMO


Kaiser Permanente is the HMO service offered by the city. Employees enrolled in the Kaiser plan, must use Kaiser or Portland Clinic facilities and doctors. Employees pay $15 co-pay per visit. This plan has an alternative care package which includes chiropractic, naturopath and acupuncture benefits with $15 co-pay and annual maximum benefit to $1500.

Prescription benefits are available at Kaiser Pharmacies; $15 co-pay for a 30-day supply. Mail order prescriptions are available; $30 co-pay for a 90-day supply.

Vision


All three medical plans include vision coverage according to an established schedule.

There is no deductible for covered services or supplies and the benefits are paid up to a maximum of $300 every calendar year for all covered services under MODA Health for all groups. For Kaiser, there is a $150 allowance every 24 months for BPA employees. Management and SEIU employees have a $300 allowance every 12 months. Payment is based on the contracted fee for in network providers and billed charges for out of network providers.

Dental Insurance


Delta Dental, also known as Oregon Dental Service (ODS), provides dental coverage for the city. Employees must use a member dentist to receive full coverage. Coverage begins the first of the month after three full months of employment. The plan is incentive based. Coverage starts at 70% the first year and increases, with annual check-ups, by 10% each year. There is no deductible. The premium is fully paid by the city. Differences in coverage are based upon the employee group as follows:
  • Management employees: Orthodontic benefit is paid at 100% up to a lifetime maximum of $1,500 per eligible employee and their covered dependents. Annual maximum for other covered services per eligible individual enrolled is $3,000.
  • SEIU employees: Orthodontic benefit is paid at 100% up to a lifetime maximum of $1,500 for dependent children if treatment begins prior to their 17th birthday. Annual maximum for other covered services per eligible individual enrolled is $2,000.
  • Beaverton Police Association (BPA) employees: Orthodontic benefit is paid at 100% up to a lifetime maximum of $1,500 for dependent children if treatment begins prior to their 17th birthday. Annual maximum for other covered services per eligible individual enrolled is $2,500.

Life Insurance


Life insurance is provided through Standard Insurance. The premium for employee basic coverage is fully paid by the city. Employees have the option to purchase supplemental insurance that offers higher limits and dependent coverage.
  • Management Employees: Coverage is for $50,000.
  • SEIU Employees: Coverage is for $50,000.
  • BPA Employees: Coverage is for $50,000.

Accidental Death and Dismemberment Insurance (AD&D)


Coverage is provided through AIG Life Insurance Company. Coverage differs by employee group. The basic premium is fully paid by the city. Employees have the option to purchase supplemental insurance that offers higher limits and dependent coverage.
  • Management Employees: Basic benefit amount is $151,000 for executive employees; $101,000 for non-executive exempt management employees and $81,000 for non-exempt management employees working 40 hours per week.
  • SEIU Employees: Basic benefit amount is $81,000 for employees working 40 hours per week.
  • BPA Employees: Basic benefit amount is $81,000 for employees working 40 hours per week.
